湾区社区卫生 was formed in 2020 off the strength of southern Alameda County’s Tri-City 健康 Center and Santa Clara County’s 丘陵地带 社区 健康 Center. Formed during the middle of the COVID-19 pandemic, 湾区社区卫生 (BACH) brings more than 70 years of combined service to the area that stretched from Union City to Gilroy. 巴赫超过100人,000人, who rely on  high-quality healthcare services, regardless of their immigration status, 种族, 残疾的人, 或者支付能力.

Valley Homeless 健康care Program (VHHP) provides healthcare services to people experiencing homelessness in Santa Clara County. We provide care to over 7,000人 every year. Our services are set up to meet the needs of the community. VHHP’s clinic locations include the Valley 健康 Center (VHC) at Alexian, 家庭第一庇护所的VHC, 卫奕信诊所, 希望诊所. We also deliver health care services on three Mobile 健康 Centers that serve many locations throughout Santa Clara County. VHHP also serves specific communities with unique healthcare needs through its Medical Respite Program, 性别诊所, Saludos Migrant Farmworker Clinic, Backpack Homeless 健康care Program, 和小货车. We also link people to additional healthcare services within the greater Santa Clara Valley Medical Center system.
